This webpage releases a sample of recent entries from the VA group called "Misc Liars." The lying is proven by video and VA documents.
These are some of the VA's itsy-bitsy liars.
The superstar liars can break the 30 lie mark... in just one medical entry. Those lies are also far more damaging to the patient's healthcare.
Video proves her lies. This person should not be employed in healthcare. "Banned for life" is the proper disciplinary action for blatant betrayal.
Their lying is supported by the Phoenix VA to the extent that healthcare workers will perform the lying WHILE the patient is complaining to them about their lying.
It is the supreme display of arrogance caused by full support from the Phoenix VA management.
VA liars need to be thoroughly investigated for their inexcusable actions. Every single liar needs to be removed from the healthcare industry for life. There should be ZERO tolerance for this behavior. What does the Phoenix VA do instead?
The entries on this page are being published as a direct result of Michael Welsh's failure to help a desperate patient with a simple request.
Ask Michael Welsh why he's incapable of acknowledging obvious issues, even when a patient repeatedly requests his assistance. Also ask him if disciplinary action was taken regarding the lying shown here. Or did he whitewash it? That is all the Phoenix VA has done in the past, when patients expose wrong doing.
Just ask him, and watch his rhetoric flow.

09-12-22 - Lorinda Evans - 6 lies - Nursing Assistant
Lorinda Evans could have typed the truth, or nothing. Instead she told five lies in three minutes.
It was her choice to put lies in my medical record. Why did she do this to me?
Lying Lorinda has proven, she should be banned from the healthcare industry for life.
Lying Lorinda... you can't be trusted. You earned a t-shirt today. I suspect you earn a t-shirt every time you open a patient's medical record.
Lie #1: "Temperature 98.6 F"
She did not take my temperature. She made up a temperature and entered it into the vitals section of my medical record.
Lie #2: "Would you like to discuss your stress level with your provider today? No"
She did not ask this question
Lie #3 location of pain: "Location: Neck, Abdomen, Arms, Legs"
This is not the answer she was given.
The actual dialog...
Lorinda: "And the pains that you're having. Where are you having them at?"
Patient: "It's muscular, all over the place and a little bit in my spine."
Lie #4: She did not ask this question.
"What # best describes the pain on average in the past week? (0 = no pain and 10 = pain as bad as the patient can image) Level(select only ONE number): 5"
Lie #5: She did not ask this question.
"What number best describes how, during the past week, pain has interfered with the patient's general activity? (0 = does not interfere and 10 = completely interferes) Level(select only ONE number): 5"
Lie #6: She did not ask this question.
"What # best describes how, during the past week, pain has interfered with the patient's enjoyment of life? (0 = does not interfere and 10 = completely interferes) Level(select only ONE number): 5"
Also... Oh boy! She put more in my record. It's not lies, just utterly STOOPID! Including answers for these questions would have been a lie because she didn't ask the questions. It's so good of her not to lie. I should be appreciative, right? Nope. Seven years of VA liars polluting my medical record is far too much. I'm not putting up with it anymore.

2. On average, how many minutes per session?"
She didn't ask these questions and didn't put lies for answers. Why did she put them in my record at all?

09-26-22 - Gloria Magallanez - 12 lies - COVID and Flu Vaccines
She gave me vaccines for COVID and the Flu. She was exceptional, especially for the VA. Very professional and efficient, kind and friendly until you see what she wrote in my medical record.
She didn't do her job. She put blatant lies in my medical record. Read what she wrote and you see the back stabbing two-faced asshole that she really is. Why did she do this to me?
I let this person stick needles in my arms and inject me with something. Does the VA think I'm stupid enough to believe this liar did that part of her job properly?
They see nothing wrong with lying in patient's medical records.

Lie #1: "The patient was given the EUA Fact Sheet for this vaccine"
She did not give me this
Lie #2: "The fact sheet was reviewed with the patient"
There was no mention of a "fact sheet"
Lie #3: "The patient was advised to remain in the facility for 15 minutes post vaccination"
She did not tell me this
Lies #4 & #5: "The patient was given a completed COVID-19 vaccination record card,a copy of the VA Side Effects and Adverse Events Reporting Fact Sheet and instructed on how to report any adverse reactions."
She updated my covid card, but did not do the rest of this statement, it is blatant lies
Lie #6: "The patient was given the influenza VIS which lists the benefits and side effects of the vaccine and which reviews the risks of not receiving the flu vaccine."
She did not give me this, I assume "VIS" is a printed handout
Lie #7: "The VIS was reviewed with the patient"
Blatant lie
Lie #8: "and they were given an opportunity to ask questions."
More lie. She said nothing about a "VIS" so clearly there was no "opportunity to ask questions" about it.
Lie #9: "The patient was provided education on how to decrease the risk of influenza infection"

Lie #10: "including social distancing"
Lie, lie... lie lie lie... it's like a nursery rhyme stuck in my head.
Lie #11: "and use of good hand hygiene."
Does this liar have at least one more lie in them? Well, certainly. The lies flow so freely.
Lie #12: "The seasonal influenza vaccine VIS given to the patient:
VIS version date Aug 6,2021."
Again, this is likely a printed handout, but I was not given ANY type of paperwork, only an update to my COVID card.
Well jeepers, we're finally at the end of her lying, for a few minutes, until her next victim.
Lying is routine. The Phoenix VA needs to answer for what they do in daily operations.
I have legitimate questions.
What else did she lie about?
What did she inject me with?
Was it expired?
Was it water?

This video was recently passed between Phoenix VA employees. A healthcare worker who saw it that day, commented on multiple procedural errors which were obvious in the video. People who lie so freely can't be trusted to do ANYTHING correctly.

Why do they LIE?
Are you dumb enough to trust a lying asshole on other parts of their job?
All she had to do was take vitals and be on her way. Like almost all VA employees, she couldn't do it without lying.
Most odd, their lies usually have no obvious purpose. There's no reason to lie... yet it's oh-so-common among VA employees.
This is a seriously bizzarroland cultural phenomenon. It is a MASSIVE problem. Anyone reading your medical record will assume the lies are true.

Lie #1: "Height 71.969 in"
This is in the vitals section, not in her notes. Only at the VA... noplace else on Earth would anyone claim to measure a person's height to a precision of 71.969 inches! Can't-stop-laughing... at the stupidity of this... especially when you know... it's a height which has been copy/pasted before...
She did not measure my height. There was no mention of height in this encounter.
I understand the assumption that a person in a wheelchair can't get up to be measured. It does not justify entering a lie in my medical record. All of the lies told in my record are accepted as truth. A patient cannot get lies removed from their record. I have tried. I've been promised by VA department heads that they would remove lies after I proved they were lies. Those lies are still in my record many years later. Lies cause healthcare problems regardless of how trivial they may seem.

This is in the vitals section, not in her notes.
She did not measure my weight. There was no mention of weight in this encounter.
Same as above, lies cause problems regardless of how trivial they may seem. If weight is trivial then why is it important enough to lie about it?
Lie #3: "Is the Veteran in pain now?
No pain now."
I said yes, she wrote no.
I was actually in so much pain that it was causing a lack of concentration. I was even lethargic and a bit confused in my answers to her questions.

Why do they lie? She didn't ask the question, so why put a lie in my medical record?
